Patients who visit one of hospitals or clinics listed below are screened for food insecurity and receive a healthy choice or diabetic-friendly food box if needed. These are especially helpful for patients who are struggling with health conditions like diabetes or high blood pressure. Some healthcare partners have an onsite food pantry or host a mobile food pantry truck onsite in partnership with the food bank.
Healthcare partners are equipped to refer patients to local food pantries and mobile food pantries and to the Food Bank’s outreach team that helps neighbors sign up for SNAP and other resources.
